What it’s for

The Rules say you must publish how people can make a request. The form is that route; the tracker logs each request the day it arrives and counts down to its target date.

What you fill in

   The request form

   A Word form for the rights under the Act: access, correction, erasure, nomination and grievances. It includes identity verification and a box for internal use.

   The tracker

   An Excel workbook with four sheets: Instructions, Settings, Rights Requests and Dashboard. Each request gets a calculated target date and a countdown, and the Dashboard shows what is open and what is urgent.
